Case Management
The Middleton Senior Center has two Case Managers on staff who are available to answer questions and refer seniors, their families and caregivers, to appropriate services available in the community. To reach a case manager call (608) 831-2373 email Jill Schonenberger or Kathy Lauer.

Information & Referrals
Case managers are a good resource for seniors, their families, and caregivers if they have questions about any of the following:
  • Medicare, Medigap, Medicaid, and other medical assistance
  • Long-term care
  • Housing – Independent Living, Assisted Living, Nursing Homes
  • Disability and Social Security
  • Food Stamps, energy services and rental assistance
  • Power of Attorney, Guardianship and referrals to benefit specialists

Program Options
Case managers can connect seniors with such programs as:
  • Durable Medical Equipment - Loan Closet
  • Meals on Wheels
  • RSVP - Transportation to medical appointments
  • Supportive home care
  • Support groups
  • Roadside Review - Safe driving options for seniors

Support Groups
  • Low Vision Support Group - This group meets monthly. Guest speakers offer solutions for individuals with low vision. Support from others who live with low vision. This group meets on the second Thursday of each month at 10 a.m.
  • Caregiver Support Group - Monthly meetings to offer support and discussion for persons caring for an older adult at home. They meet on the second Thursday of the month at 1 p.m.
  • Essential Tremor Support Group - This group meets monthly on the second Wednesday of each month at 10:30 a.m.

Health
  • Blood Pressure Screenings – Volunteer nurses provide blood pressure screening monthly on the first Tuesday of each month beginning at 12:30 p.m. No appointments are necessary.
  • Foot Care - The Middleton Senior Center offers foot care in collaboration with Home Health United. The cost is $20. Diabetic foot care cost is $28. Please contact the Senior Center at (608) 831-2373 to schedule your appointment.
  • Financial Assistance (Taxes) - The Middleton Senior Center, in collaboration with AARP, offers free income tax assistance during tax season for seniors and those with low to moderate income. Appointments are required.
  • Medicare Part D - Navigating the decisions related to Medicare Part D plans can be complicated. Volunteers are available during the Part D enrollment period to assist seniors in choosing the best plan for them. Medicare Part D plans change annually and it is best for everyone to evaluate their options every year. Appointments are required.
