  • Daily Enrichment Activities including art, sports, gardening, music, drama, science, photography, cooking, Girls Group, and more!

ENROLLMENT PACKET

* Please submit the required forms by the first day your child attends the Middleton Youth Center.  The enrollment packet contains an enrollment form, code of conduct, and parent authorization form. Enrollment packets are available at the Youth Center during operating hours and below:

Enrollment Packet (English)
Enrollment Packet (Spanish)

Bus Form (This form is only required if you want your student to be able to ride the bus from Kromrey to the YC after school. The form should be submitted to the MCPASD Transportation Department.)

VOLUNTEERING

The two main volunteer positions we have available are:

  • Tutors for the Homework Help Room: (M-R 4:15-6pm)  Tutors work with students one-on-one or in small groups, and help keep the homework room a quiet, productive space.  We ask that all tutors are able to help with 5th-8th grade level homework in all core subject areas.
  • Weekly Activity Leaders: (M-R 4:15-5:15pm)  This is usually a better opportunity for college students or older.  We ask that activity leaders commit to planning and leading a 1-hour long activity at the same time once per week for at least one semester.  For example, leading a weekly art class.

To be considered for a volunteer position, please submit the following forms to :
